Job Summary

The Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team. The RN provides direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment. These three factors have an impact on decision making related to care-provider assignment as well as the need for consultation and collaboration among care providers. All nurses make a commitment to lifelong learning.

The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible to provide quality care to residents with diverse care needs in long term care with diverse care needs.

The RN is a unique position that involves partnership with Nurse Practitioners (NLOT) to ensure timely access to high-quality care for Long Term Care (LTC) residents.

The RN will collaborate with the NPs to provide expert clinical care, mentorship and support to primary clinicians, nurses, and other members of the health care team to enhance clinical best practices for the elderly in LTC with the goals of:

• Reducing Hospital admissions and emergency department transfers for conditions treatable in LTC
• Shortening Length of stay for residents in acute care who can safely be transferred back to LTC with appropriate support from clinical teams (Primary care providers & NP)
• Helping to increase capacity of LTC staff to detect and manage acute changes in resident conditions

Primary Responsibilities

• Collaborates with the NLOT Nurse Practitioners and all members of the LTC health care team.
• Provides patient and family focused care within the RN scope of practice.
• Functions in accordance with established hospital policies and procedures, including emergency codes
• Ensures that practice is consistent with entry to practice competencies, standards of practice, guidelines and legislation.
• Provides face to face or virtual visits in long term care homes.
• Communicates effectively with residents, families and all members NLOT of the health care teams.
• Supports implementation, recommendations or treatment plans established in consultation with the primary care provider/NP.
• Provides consultation and/or clinical support to health care professionals and peers based on best practices.
• Performs a detailed assessment of a resident's health status when requested and documents within CNO guidelines
• Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.
• Assists in the orientation/preceptorship of new staff members and students to the clinical team.
• Acts as a role model and champion to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care.
• Effectively utilizes human and material resources.
• Participates in the development, implementation and evaluation of clinical pathways and educational modules.
• Other duties as assigned by Manager/delegate.

Qualifications

• Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
• Bachelor’s Degree in Nursing preferred
• Minimum of two (2) years of direct clinical experience in Medicine, Surgery, critical care or the equivalent within the last 5 years
• Must possess own vehicle, a current Ontario Driver’s Licence and a clean driving record
• Ability to maintain a private and secure home office.
• Ability to work both independently and cooperatively with other members of the community-based interprofessional and primary care teams.
• Self-directed and able to manage multiple priorities
• Problem solving, critical thinking and analytical skills
• Demonstrated knowledge base of pharmacological and non- pharmacological interventions for management of chronic conditions
• Excellent physical assessment skills
• Excellent computer literacy – Microsoft Programs, e.g. Outlook, Word, PowerPoint
• Broad understanding of the health care system including Home and Community Support Services, acute care hospitals and Community Organizations e.g. hospices
• Innovative and willing to learn.
• Demonstrates initiative and the ability to function both independently and as an interprofessional team member
• Excellent interpersonal, communication and public relations skills.
• Demonstrates excellent clinical skills and professional leadership capabilities with a focus on collaborative teamwork, patient flow, and quality improvements.
• Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
• Good working knowledge of Electronic Health Care Documentation.

As a condition of employment, you are required to submit proof of COVID-19 vaccination to the Hospital’s Occupational Health and Safety department.
