Job Summary

The Nurse Practitioner with the Nurse Led Outreach Team uses advanced practice clinical skills to provide direct clinical care, consultation and treatment in the area of Geriatrics to support the acute care needs of residents in LTCHs. They are also to provide education, mentoring and coaching to front line staff to help build capacity for the LTCH within the Northern CLHIN catchment area. To participate in research and quality improvement.

Qualifications

• Master’s Degree in Nursing/Nurse Practitioner Stream, or Master’s Degree in Nursing with clinical expertise in Gerontology, or Masters’ in Nursing/Master of Science in Nursing with successful completion of a post Masters Acute Care Nurse Practitioner Certificate program or commitment to pursue within a specified timeframe.
• Registered Nurse in the extended class with the College of Nurses
• Gerontology Nursing Certification
• BCLS required
• Valid driver’s license
• Minimum 3 years’ experience in advanced practice nursing in geriatrics/eldercare and clinical specialty-related experience as a Registered Nurse Demonstrated teaching skills
• Demonstrated program development skills and experience
• Knowledge and understanding of the health care system, LTCHs and care options for the elderly.
• Demonstrated commitment to excellence in patient/family driven care.
• Demonstrated ability to work collaboratively in an inter-professional team environment, conveying a positive and professional attitude.
• Recent experience in a leadership role within an inter-professional team
• Relevant knowledge of LTCH environment, hospital and community resources
• Demonstrated superior organizational, administrative, and problem-solving skills.
• Capacity building, leadership and facilitation skills.
• Excellent organizational and time management skills.
• Superior interpersonal and written and verbal communication skills.
• Nurse Practitioner’s Association of Ontario.
• Computer literacy
• Bilingualism in French/English an asset.

As a condition of employment, you are required to submit proof of COVID-19 vaccination to the Hospital’s Occupational Health and Safety department. Successful candidates will be required to submit proof of COVID-19 vaccination (two doses) from the Ontario Ministry of Health website or other authorized source directly to the Occupational Health and Safety department.
